Client Intake Forms

In addition to completing the form(s) below, it is the client's responsibility to view and familiarize oneself with all spa policies. Click here to view Policies in a separate window.

All clients must complete and submit the appropriate form(s) prior to the scheduled service(s). Please click on the link(s) for the appropriate form(s) for each type of service you will receive. If completing multiple forms, it is recommended to open them in separate tabs simultaneously so that you can Copy + Paste some of the required fields that are the same across all forms. New forms must be completed with up-to-date information on an annual basis by all clients. Forms are secured by Jotform protected by a 256 bit SSL connection using a SHA256 Certificate with an additional layer of encryption so they can only be accessed by Lisa Parnell, owner of Lotus Organics Day Spa. Data will never be shared for any reason without client consent.
